About Us

Trinity Oaks Continuum is designed for senior adults: apartment living, cottage homes, light personal care apartments, wellness programs, pool and spa. We also have a Lutheran home with assisted living, intermediate care, rehabilitative services and skilled care, Our other services include: adult day care, chaplaincy services, in-house pharmacy, scheduled transportation, social worker on staff and support group for families. Some nursing services are Medicare/ Medicaid approved.
